*Seminar Registration Form and Fee must be received in advance for proper planning. Please bring your ID to check in at the door. No one other than approved attendees will be permitted inside the training room. No photos or recordings without prior approval by Tim Lewis of LCI Services.

*If a cancellation is requested by the attendee or agency up to four weeks before the event/class, the amount will be refunded, less a $150.00 service fee. No refunds after that.

*Payment is due by check-in on the first morning of the class. Late Payments not received within 30 days or the first day of the training will be charged a 20% monthly interest fee.
